Client/Applicant Agreement:

You agree to allow agency to submit this application via electronic, directmail, and/or overnight delivery to the qualifying and/or approving lender for the Prepaid Credit Card and/or the Secured Credit Card as you have selected on this application.  You understand that all monies, deposits, and/or fees paid to the Agency for securing your Prepaid Credit Card, and/or your Secured Credit Card are non-refundable, and your account will be billed like all other credit cards each month as you utilize the assigned credit limit, which is determined by the amount of money in which you load and/or deposit to either card.

The PrePaid Mastercard will require a monthly payment of $11.00, which is billed on the Prepaid Credit Card each month, normally around the 2nd of each month and consecutively thereafter for a period of two (2) years, and/or twenty-four (24) months.  You agree to ensure that monthly payment minimums as stated are loaded on the Prepaid Credit Card each month in order for your new account to continue reporting to all three (3) major credit reporting agencies.  The maximum load on the PrePaid Mastercard is $2,500.00 and/or a maximum of $300.00 per day up to the maximum load value of $2,500.00.  Your credit limit reported to all three (3) credit reporting agencies shall remain at the exact amount in which you have loaded onto the card at any given time.  You may increase the amounts anytime by making deposits to the card by mail, bank wire transfer and/or by money order or personal check at the address provided to you when you receive your card.  You understand that allowing your Prepaid Credit Card to reach a minimum balance of Zero (0) shall constitute your breach of your agreement to continue the account for a period of twenty-four (24) months, and the bank issuing the card may decline you from further use of the card, and/or close your account at their discretion when you reach a balance of Zero (0) at any given time.  This Prepaid Credit Card will eventually become an unsecured credit card and will carry a maximum credit limit of $20,000.00 when you have successfully qualified for additional credit by using your card wisely and not allowing it to make the balance of Zero (0).  Annual participation fee for this Prepaid Credit Card is $99.99 and will be due each year that you renew, activate and/or continue to carry this card.  The acceptance, account setup fee is $300.00 when referred by a Broker of our agency, and $500.00 when you have not been referred by someone.  Total amount required must be paid by Cash through the Broker or agency providing this application, and the $99.99 annual participation fee must be debited on an existing credit card and/or from your existing personal checking account.

The Secured Credit Card - will provide you with the same privileges of the Prepaid Credit Card, with the exception that this Credit Card will be secured for only one (1) full year and $200.00 of your deposited proceeds into a Savings Account will be refunded to you immediately upon your anniversary date, based on the manner in which you have held your account with the issuing bank.  If your account has not been maintained in an excellent fashion, your secured funds will remain on deposit and returned to you at a later date when your account has meet the minimum requirements of unsecured status.  The Secured Credit Card will also report to all three (3) major credit reporting agencies and will carry an annual participation fee of $99.99 upon your acceptance.

Signature and Acceptance:

Your signature below confirms that you are requesting this agency to submit your request for an immediate approval for a Secured Credit Card and/or a PrePaid Mastercard as outlined.  You authorize agency to make debits into your personal checking account and/or to debit your credit card for your annual participation fees required to obtain these cards.  You understand that your application and account setup fees totaling $300.00 are required to execute this request for a PrePaid Mastercard, and a minimum of $500.00 is required to execute your requests immediately for a Secured Credit Card.  Sign your signature below and return your payment immediately to the Agent, Consultant and/or Representative providing this application.

You will receive your new card within 5-10 business days from the date of this application.  If for any reason you are denied the Secured Credit Card, your application will be processed and submitted for the PrePaid Mastercard for your immediate use when received.
