YOU MUST ATTACH A COPY OR RETURN A VOIDED CHECK WITH YOUR ORDER. ALL COPIES MUST BE LEGIBLE, CLEAR AND EASY TO READ. PLEASE MAKE SURE YOUR COPY QUALITY IS LEGIBLE BEFORE FAXING, EMAILING, AND/OR SENDING TO US BY DIRECTMAIL. WE CANNOT PROCESS YOUR ORDER WITHOUT YOUR VOIDED CHECK.

(First Photograph impression on checks is "Free" at no charge. $12.50 photo charge for Second and Continuous photographs on check orders.) (Add $18.50 for verification of all credit card payments, and $22.50 if using a Bank Debit Card.) We reserve the right to decline any picture or photograph submitted that is distasteful, bogus, and/or obscene and will destroy all elicit and X-rated pictures when received. Photographs may be sent in by overnight delivery for impression capturing for check layout, and will be returned to customer completing order to the address imprinted on your new checks. If you require a proof of your check order BEFORE WE IMPRINT THEM AND PREPARE YOUR FINAL CHECK ORDER, please sign below, and enclose your Check Proof Fee of $25.00 + $18.00 Shipping and Handling for next day and/or two-day delivery.

5.	Original artwork release required:
	Legally, we cannot scan any material that has been published or that is copyrighted. Cartoons, images from magazine advertisements, articles or other company check designs, are prime examples of copyrighted material. If you are not the owner of an image or photograph that you would like to use, then you must get a signed Release Form and Disclosure Statement before we may use them. Artistic! Check Products and Checks Your Way, Inc. will provide examples as a courtesy, but DO NOT support copyrighted material when placing information onto check designs. We always accept ORIGINAL artwork not related to copyrighted material. Images submitted for consideration must not contain or depict profanity, nudity, violence, or be generally unacceptable among the public at large. Artistic! Check Products and Checks Your Way, Inc. reserves the right to refuse to print any image or photograph or other material for any reason, and may return your Order if all required information is not provided.
