To make a payment by FAX, follow these instructions:

2. DO NOT VOID YOUR CHECK
3. We will create a draft of your check for depositing and your check
may be used for your records.
4. If your check has pictures, images, and etc. on the face of it, please
enlarge your check for clarification of information.
5. Complete our Order Form or Application from any of our websites for
the product, program, or service you are interested in and return with
payment and application as required.

To make a payment by CREDIT CARD or ONLINE, follow these instructions:

1. Send an email to us requesting a payment form. All payment form requests
should be directed to: payments@artistic-capitalcorp.com.
2. YOU MUST COMPLETE OUR PAYMENT FORM IN ORDER TO SEND
YOUR PAYMENT BY CREDIT CARD OR CHECK ONLINE.
3. When you have received confirmation that your payment has been
received, send your Order Form or Application to us by email, fax, or
directmail for immediate processing.
4. You will receive confirmation from us before we process your
request.
5. We require your original signature on payment form for all check payments
by fax and/or email requests.
* Effective January 1st, 2004 our agency will not be accepting major credit cards for any
