                     Client/Applicant Agreement:
You agree to allow agency to submit this application via electronic, directmail, and/or overnight delivery to any and/or all affiliated partners, associates, brokers, and/or individuals who is within the Artistic! Capital Corporation's Network of Credit Enhancement organizations.  If none are interested in the offer, to service the request as noted; you will be serviced by the agency, Artistic! Capital Corporation.

Verification/Certification: You certify under the penalties of perjury that you are the named individual on the application and that you have not assumed someone's personal identity and/or credit information in submitting this request to the agency.  You agree to provide the agency with any and/or all other information that may be required if requested to process your request amicably.  You understand that all information submitted to the agency shall be held in the strictest of confidence and at no time during the servicing of your request can you provide to any others any information regarding the agency and it's association with you and/or the services in which they have or have not provided to you directly and/or indirectly.  You confirm that you are not associated, affiliated, employed by and/or working with any law enforcement agencies to learn the business affairs of this agency and its services offered to consumers.  You verify that you are at least 18 years of age or older and that you willingly contacted to this agency for the services in which you are seeking; and you were not solicited by this agency for any service requests.  You understand that there are no refunds allowed at anytime once you submit your application for processing, when it is accompanied by your payment, whether by Cashier's Check and/or US Postal Service Money Order.  You understand that all sales and services are final and refunds are not allowed.  You understand that you are solely responsible for any and/or all debts, finances, credit or any other credit or financial related matters that transpire once your new credit file has been provided to you by the agency.  You understand that all debts shall remain yours and the agency is not and cannot be held accountable for any actions you take individually, jointly and/or mutually with others once you receive your new credit file and begin using it.  You also understand that the agency shall not release to you your new credit file until your entire balance owing has been paid-in-full and/or upon which services shall be considered rendered when the agency has delivered you your new credit file and your payment has been made accordingly.  You understand that the agency shall charge you an account setup fee of $250.00 for each new account that is setup for you while requesting a new credit file; as well as charge you accordingly for any and/or all processing fees sustained by the agency by any participating service provider, agency, credit builder representative and/or outsourced agency if used.  You agree to pay to the agency immediately any and/or all outstanding invoices due for your account setup.  You understand that your new credit file will take a minimum of thirty (30) business days to complete and/or a maximum of thirty (30) to ninety (90) business days if you are requesting a new credit file with a guaranteed fico score.  You also understand that the agency is not responsible for the acceptance of information or not by any credit reporting agencies.  If your data is transmitted once setup has taken place, you understand that the agency is not responsible for any acts, actions, or rejections made by any and/or all credit report agencies should they refuse your new credit file once submitted.  You agree to hold the agency harmless for any actions made or not made by the credit reporting agencies should they reject or refuse your submitted data for new credit file as purchased.

NON-CIRCUMVENTION - You agree NOT to use the services of others that are similarly related to the same services extended to you by this agency while you are requesting services.  You agree to not release any and/or all documentation received from this agency by you to any other parties, sources, individuals, agencies, and/or all other law enforcements not mentioned.  You understand that all rights of ownership of the documentation you receive shall remain solely to you when released to you by the agency and you shall be solely held accountable by the agency should you wrongfully misuse any information received and provided by the agency to you.  You agree to not disclose any information regarding your association with this agency to any other parties unless you have requested written confirmation by an agency officer that you may do so.  Your signature and acceptance below is your original signature and none other.  When your signature is placed on this agreement, and your application is returned to us by fax, email or directmail by express services; we shall consider your application for processing.  You understand that we reserve the right to accept, decline, refuse, or terminate your application at any time if we should desire to, and we are not obligated to  provide you the services as requested if we feel at any time that you have violated our Noncircumvention Agreement and statements made herein.  Place your signature and date on this application, complete the application and return with your payment for immediate processing.

OVERNIGHT EXPRESS OR PRIORITY MAIL DELIVERY:  You also understand that we will refuse your application and payment for service if you return your request by any other delivery methods other than Overnight Express Airmail and/or US Postal Service Priority Mail.  We only accept these forms of delivery to our corporate address below and will refuse all other packages and/or requests when received.  We are not to be held accountable once we refuse your package.  You are responsible for the appropriate delivery methods requested if you wish to use our services.

BEFORE MAILING YOUR APPLICATION:  Fax us a copy of your completed application and your payment to: 1-866-RTISTIC! (1-866-784-7842) for our records.  We will immediately begin to work your new file request upon your faxed verification.  This will speed up the processing time by two (2) business days if you fax your request to us before overnighting it or mailing it.

Signature and Acceptance:

Your signature below confirms that you are requesting this agency to consider your request for a CreditBuilder Account for either a new credit file setup and activation and/or a new credit file with a guaranteed fico score of 700 or higher.  You understand that your application and account setup fees will total $500.00; which must be added to your total amount due from above.  If not received with your application and original payment, your application will be refused.  Your signature below confirms that you have reviewed this agreement, and will abide by all terms and conditions as it has been written.
